Whats the smallest thigh it'll fit?

It should fit just about any thigh. I have a large cyclist thigh and it fits. The straps are Velcro so adjustment is easy.

would it help with night cramps.

I'm not sure but cramps could also be a sign of dehydration or some sort of agitation of the sciatic nerve in the lower back. Consider I nquiring about stim' and chiropractic adjustment treatment with a physiotherapist and ask if this hamstring support would help you.
